What would you like to do?
In choosing to complete the Personalisation Self-Assessment Tool, you have 2 options:
- Complete the assessment anonymously - This will provide a web based report at the end, but you won't be able to save it.
- Register, logon, then complete the assessment - Register as a new user and then logon. This will produce an enhanced report, available as a formatted downloadable document. Your results will be saved and available at a later date, should you wish to revise your answers, or to re-audit your status.
- NB: If you have already registered as a user, please go to the home page and logon.
How does it work?
For each line, choose the statement that best reflects your current position. At the end of the questionnaire a summary statement will be produced for your council which will offer you
- an interpretation of where you are in transforming your local systems along a recommended programme management process
- suggested next steps and resources which can support your council and partner organisations.
This exercise is intended to provide you with a baseline for your council to build on, and will prove most helpful if you are realistic about the current position of the council. The information collected may be used to shape national and regional support programmes available from CSIP and its partners, but will not be used in any performance management related context. The exercise assumes that the task ahead of you is one of transforming all your adult social care system, and so it is not broken down into different 'client group' areas, although it is recognised that there is likely to be significant variation between different service areas and that this will be part of your challenge. We are also assuming that your priority task will be to create a system based on personal budgets made up of social care funding - although we recognise you may decide to bring in other funding streams, including increasing integration with health resources.
